What is liposuction?
Liposuction (also called “lipo”) is the process by which a skilled surgeon removes excess fat beneath the skin (subcutaneous). The fat is suctioned from an incision or series of incisions to both remove bulk and sculpt the area for a more contoured physique.
Is liposuction surgery?
Yes. Lipo is a surgical procedure. Even the least invasive techniques are considered surgery. This is because incisions are created in the skin, allowing for a cannula to be inserted to aspirate fat cells. Anesthesia is used, often locally, to prevent pain. However, some patients may also be heavily sedated via general anesthesia (rare).
Is liposuction safe?
Yes. Liposuction is considered safe for patients who are in relatively good health. This includes patients who are at a healthy weight, live an active lifestyle, and eat well. However, some medical conditions and prescriptions may prevent a safe procedure for some patients. A full consultation with a dermatologic surgeon will determine if lipo is safe for you.
What is tumescent liposuction?
Tumescent liposuction is a type of liposuction surgery that utilizes a tumescent formula for anesthesia. Tumescent anesthesia is local and contains both the diluted anesthetic and epinephrine. The anesthetic numbs the treatment area to prevent pain while the epinephrine reduces blood loss and bruising. Patients are awake during the tumescent procedure, as this is considered safer than putting the patient under general anesthesia or heavy sedation.
What areas can be treated using liposuction?
Most areas that contain excess pockets of unwanted fat just beneath the skin can be treated using lipo. Treatment areas include large areas such as the thighs and abdominal region as well as smaller areas like the double chin and knees.
How much does liposuction cost?
Liposuction cost varies depending on a variety of factors, such as the size of the area treated, the amount of fat being aspirated, and how much tumescent anesthesia you’ll require. Who is a good candidate for liposuction?
A good candidate for lipo is a patient who is in good health and already at a healthy weight. If you are within your ideal weight range, exercise regularly, and eat healthy, then you will likely see good results from a safe lipo procedure.
Can I have liposuction if I’m overweight/obese?
No. Liposuction is not a weight loss tool. Patients who are overweight or obese should consult with a physician to come up with a strategy for weight loss before considering lipo. It is a great procedure if you have lost a lot of weight recently and need help with the stubborn areas of fat that won’t go away, however. So come back and see us after you’ve lost the weight and feel healthy.
Who shouldn’t get liposuction?
If you are overweight or obese, then liposuction is not for you. If you take blood thinners or NSAIDS, it is not recommended that you get lipo. Patients with hypertension are also not considered ideal candidates. If you are underage, liposuction is generally not recommended as you’re still growing and have what is affectionately called “baby fat” that will thin out as you reach adulthood. Is abdominal liposuction like a tummy tuck?
No. Lipo is fat removal while a tummy tuck, or abdominoplasty, is skin removal and tightening. While you can have both on your abdomen, they are not the same procedure.
What will my skin look like after liposuction?
For some patients, depending on how much fat is removed from one area, the skin may appear loose. However, in the hands of a skilled dermatologic surgeon, you should not have excess or sagging skin following your procedure. It’s best to discuss your unique situation during a consultation to make sure that your needs are appropriately met.
What about non-surgical liposuction alternatives?
Some patients can avoid surgery and still lose unwanted fat with non-surgical body sculpting treatments like CoolSculpting, offered here at the Laser and Skin Surgery Center of Indiana. Many areas can be treated using CoolSculpting, but not all patients are a good fit for non-surgical treatments. Depending on your needs, liposuction may be the procedure that will provide you with the best results. What are the risks associated with liposuction?
Liposuction is surgery, so there are always going to be risks. However, choosing a board-certified dermatologic surgeon with an excellent reputation for patient safety and results will minimize the risk of side-effects and complications. Also, being sure to follow pre- and post-procedure instruction is paramount to getting the best results and avoiding unwanted side-effects.
Risks involved with lipo are similar to other surgeries. However, by eliminating general anesthesia and heavy sedation, many other risks are eliminated. As with any surgery, post-procedure bruising and discomfort are common. More moderate-to-severe complications are rare and include infection, excess bleeding, and poor results. Again, following your dermatologic surgeon’s instructions and selecting a reputable surgeon are paramount to avoiding such complications.
What is the downtime following liposuction? How is pain and discomfort managed?
With the tumescent procedure, downtime is significantly reduced compared to more traditional lipo involving general anesthesia. Bruising is nearly gone after 1-2 weeks. Patients may resume exercise as early as 3-7 days following the procedure, although Dr. Hanke may recommend a longer downtime for you. Most patients may return to work as early as 3 days following tumescent liposuction, as long as the demands of the job do not include activities such as heavy lifting or excessive bending and twisting.
For the first 24 hours following your lipo procedure, the tumescent anesthetic is still active and managing pain and discomfort. After the tumescent anesthetic wears off, most patients only require over-the-counter acetaminophen to manage any discomfort.
Will my procedure be performed in a hospital? Will I have to stay overnight?
No. Your procedure will be performed in a surgical center. A hospital is not required for tumescent liposuction, nor is an overnight stay. Most patients are able to leave (with the help of a caregiver who will transport them home) within 30-60 minutes following surgery.
How should I prepare for my liposuction procedure?
For best results, it’s important to prepare far ahead of time for your procedure. These steps help ensure your safety during surgery and improve outcomes. You will receive a packet that includes your full pre- and post-operative care instructions. Some of the pre-procedure instructions include the following:
- do not eat or drink after midnight the night before your procedure
- quit smoking at least 2 weeks prior to surgery
- do not shave ahead of your surgery to avoid ingrown hairs
- wear loose fitting clothing the day of surgery
- find a friend or family member who you can count on to transport you to and from your appointment

Liposuction is surgery. However, with the tumescent procedure, patients are not put under general anesthesia. This improves patient safety dramatically. During tumescent liposuction, tumescent local anesthesia is used to numb the treatment area. After making small incisions, your dermatologic surgeon inserts a cannula to remove unwanted fat.
Liposuction isn’t simply a vacuum to remove fat; it is a contouring procedure that helps reshape areas of the body. In the case of buttocks, removing excess fat surrounding the buttocks helps to naturally enhance your derriere without the need for butt implants. Recovery time is shorter and less intense as well, and the procedure is safer without general anesthesia or a foreign object being implanted into the body.

CoolSculpting is not approved by the FDA for application directly onto the buttocks; it is approved just beneath the buttocks (banana roll) and the inner and outer thighs, as well as for fat reduction on the abdomen, flanks, and back/bra fat.

What about exercising to enhance by buttock size?
Following your reshaping procedure, exercise can help enhance the gluteal muscles for an enhanced backside without butt implants. Who Shouldn’t Get CoolSculpting?
There are only a few reasons why CoolSculpting may not be right for you. Be sure to discuss any of these or other concerns with your providers before proceeding with treatment. CoolSculpting is not a weight loss tool and is not a good fit if you are overweight or obese. It is also not appropriate if you are looking for immediate results. You are not a good candidate if you are pregnant or breastfeeding or if you suffer from chronic pain, chronic fatigue syndrome or an autoimmune disease. While CoolSculpting and liposuction help provide similar results in reducing unwanted body fat, that is where their similarities end. Unlike CoolSculpting, tumescent liposuction is a surgical procedure and can be used almost anywhere that fat collects around the body. However, don’t let the fact that this is a surgical procedure scare you. Tumescent liposuction is one of the safest cosmetic surgeries available today.
Tumescent liposuction is performed under local anesthesia, meaning you will be awake during the liposuction procedure. Prior to incision, Dr. Hanke will inject a solution containing lidocaine for pain relief and epinephrine for vasoconstriction; vasoconstriction is the constriction of blood vessels to minimize blood loss during the procedure. Up to eight tiny incisions will be made in the treatment area. Dr. Hanke will use a cannula to suction out the unwanted fat through these small incisions. Following the procedure, most patients are able to return home the same day to recuperate.
What Areas Can Be Treated With Tumescent Liposuction?
You can have liposuction just about anywhere on the body. This includes difficult areas that aren’t well treated using non-surgical methods or where our CoolSculpting applicators cannot be effectively applied.
Who Can Get Tumescent Liposuction?
While tumescent liposuction is not a weight loss solution, it is more helpful than CoolSculpting for larger amounts of body contouring. Therefore, if you have multiple inches of fat to lose from your waist or hips, tumescent liposuction may be the right choice.
The safety of tumescent liposuction has been well studied. Clinical research has shown that tumescent liposuction is very safe. In fact, in a study with over 15,000 patients, only minor complications were seen on a rare basis. No patients needed blood transfusions, and no one was admitted to the hospital.
Who Shouldn’t Get Tumescent Liposuction?
There are some reasons why tumescent liposuction may not be right for you, and you should approach these concerns honestly with your dermatologic surgeon as you make the choice between CoolSculpting and liposuction. For example, if you are not healthy and struggle with one or more chronic diseases, tumescent liposuction may be more than your body can handle. In this case, you may be at a higher risk of developing surgical complications, such as poor healing, infection or blood clots. Additionally, because it is an invasive procedure, you should not undergo liposuction if you are on blood-thinning medications, such as warfarin or aspirin. Finally, if your skin is very thin and crepe-like, you may not experience aesthetically pleasing results because your skin will not readily snap back after fat is removed.
Body Sculpting is NOT Weight Loss
When choosing between CoolSculpting and liposuction, keep in mind that both will significantly reduce the number of fat cells in your body. This is very different from the process of losing weight. When you lose weight through diet and exercise, your fat cells shrink. When you undergo CoolSculpting or liposuction, fat cells are removed. These fat cells do not return. That doesn’t mean you can’t gain weight again, however. The remaining fat cells can expand, so it’s important to maintain your healthy lifestyle following a CoolSculpting or liposuction procedure.
Should You Choose CoolSculpting or Liposuction?

Even though our bodies may showcase an hourglass figure, that pesky double chin can continue to haunt us. So what causes a double chin, and how do you get rid of it?
What Causes a Double Chin?
Technically, double chins are caused by a layer of fat that gathers around the neck and forces the skin to sag. Some families are predisposed to double chins through genetics, which can affect where fat is stored in the body or determine the bone structure of your chin, while others develop a double chin due to weight gain. For some, even after weight loss, the double chin doesn’t go away.
How to Get Rid of Double Chin Fat

Double Chin Reduction With Liposuction
Liposuction is the process of removing fat cells from areas of the body that don’t respond to diet and exercise. A dermatologic surgeon makes a small incision in the desired area and uses a tube called a cannula to suction out the pockets of fat. This is done using tumescent local anesthesia. The patient is totally awake and conversant throughout the procedure. Because this is a surgical procedure, your recovery time depends on your overall health and can last up to two weeks. You can usually resume normal activities within a week of having a tumescent liposuction procedure, which is our preferred method here at the LASSI.
Kybella: The First Non-Surgical Double Chin Eliminator
If you are not a good candidate for liposuction, or you prefer a non-surgical alternative, Kybella may be an option for you. Kybella is the first non-surgical, FDA-approved double chin eliminator designed to naturally break down fat cells, which are then removed from the body through the lymphatic system. The procedure includes injections of deoxycholic acid, a substance identical to the acid our bodies already use to break down fat cells for absorption.
Kybella treatments take about 20 to 30 minutes, during which time your dermatologic surgeon will use a small-gauge needle to inject the substance into the fat layer. Depending on the size of the treatment area, you could receive anywhere from 1-5 or more painless injections to ensure full coverage. You may also need between three and six sessions. Patients may experience slight swelling and tenderness at the injection site, but this discomfort is easily managed without prescription pain killers.
Freeze Away the Double Chin With CoolSculpting’s CoolMini
A non-invasive, FDA-approved alternative also exists for double chins called CoolSculpting Mini, a controlled cooling technology referred to as cryolipolysis that gently cools the fat cells without damaging surrounding tissues. A small vacuum applicator called CoolMini is placed on the area, and the skin is brought into contact with the cooling panels. The cold creates an anesthetic effect on the area while the fat is cooled over an hour to a fat-freezing temperature. The frozen fat cells are then absorbed by the body and removed, never to return.
This technique has no recovery time. You may experience redness or slight bruising in the area treated, but those effects will be gone within a few hours. You may need multiple treatments, but you will see a noticeable difference in two to three months.

Tummy tucks provide a trim and toned core, but the recovery time is much longer than liposuction alone. Recovery time varies from patient to patient, but you can expect complete recovery to take four weeks or more.

Tummy tucks and liposuction tend to go hand-in-hand. If you have a lot of loose, sagging skin and excess fat, having both procedures done at the same time can help tackle both problems simultaneously. In fact, tummy tucks use liposuction to eliminate the excess fat before the surgeon can remove the excess skin and help sculpt and tighten any loose abdominal muscles. Most patients do not require both procedures, which is why it is so important to discuss your concerns with the surgeon beforehand. Tumescent Liposuction involves a dilute solution of local anesthesia that is injected into the fat beneath the skin. This area becomes swollen and firm, or tumescent. The two main components of the tumescent solution are epinephrine and lidocaine.Liposuction Epinephrine causes contraction of the muscular walls in blood vessels, leading to a narrowing of the blood vessels. This effect of the drug almost eliminates all surgical bleeding during liposuction. The other component, lidocaine, provides strong local anesthesia and eliminates the need for IV sedation or general anesthesia. This method eliminates the risks associated with general anesthesia and excessive surgical bleeding. The tumescent technique solves the concerns and dangers associated with the early form of liposuction.
Before the tumescent liposuction technique was created by Dr. Jeffrey Klein, a dermatologic surgeon, liposuction required blood transfusions because blood loss was so high. Some doctors would require their patients to donate their own blood several weeks before surgery, to later be transfused during surgery. Patients who receive liposuction with the tumescent technique lose approximately 15 to 30 ml of blood. Routine laboratory studies require the same amount of blood to be taken. Today, blood transfusions are no longer a necessity in liposuction surgery.
The excessive bruising associated with liposuction is considerably reduced with the tumescent technique. This is because of the minimal amount of blood loss linked with the tumescent technique and because of the amount of drainage of blood after the operation. The small incisions do not require stitches and are left open to allow the remaining mixture of blood and anesthetic to drain.
The advantages of the tumescent technique include the following:
- Minimal blood loss
- Lidocaine can be used safely up to 45 mg/kg
- Lidocaine is suctioned out with the fat
- The effects of the anesthetic may last as long as 24-36 hours
- The swelling can highlight defects that need to be removed
- Blood transfusions are not required
- Lidocaine may decrease the risk of infections
- Post-operative bruising is reduced
