Appointments

To schedule an appointment, please call 317.660.4900. Our staff is happy to assist you! Please bring the following items to your appointment:

• A complete list of the medications (prescription and non-prescription) you are currently taking
• Your most recent insurance card(s)
• A referral from your primary care physician, if necessary/required by your insurance plan

*If you need to leave us a message during our regular office hours, your phone call will be returned promptly. After hours, your call will be returned the next business day. If an emergency arises after hours, please contact the Laser and Skin Surgery Center of Indiana via the phone number provided on the voice mail or go to the nearest emergency room.
